
Effective March 15, 2006, all Hazardous Materials Business Plans (HMBPs) and Registration (‘Short’) Forms must be submitted electronically via the Unidocs website (www.unidocs.org) or by submitting via email or CD an Excel or tab-delimited format provided by the department. This will allow us to provide critical information to emergency personnel more effectively. If you submit a paper HMBP or Registration Form, we will enter the information into Unidocs and charge you for the data entry costs. The current fee is $40/hour with a one hour minimum. In most cases, previous information for your facility has already been entered into Unidocs so you may only need to update that information. In all cases a signed copy of the Owner/Operator form must be dated and submitted.
Instructions Online Submission/Updating (preferred method)
- Start by going to the Unidocs website (www.unidocs.org).
- Login (and getting a password for new users). Select the ‘login’ link under Interactive Hazardous Materials Reporting.
Enter your name and password.
If you are a new user, select the ‘create new account’ link and complete the form. Identify yourself as a business user and indicate in the text field that you want to update the hazardous materials information for your site(s). List the site name(s) and address(es) please. Follow the instructions on the website to complete the request. This might take one or two minutes. You will see a notice that your access must be verified prior to approval so you may have to wait a day or so.
- Find your site(s) Once your password has been approved, you can login to the Interactive Hazardous Materials Reporting section of Unidocs and search for your site(s).
Search for your site by ID # or business name. For ID number, you need to know the six digit number assigned by the Fire Department which can be found on your permit or invoice for annual storage permit fees (the format is 43-06-xxxxxx). Tip: If you want to search for your site by business name, use a partial business name and an asterisk instead of entering your full business name. (ex: for Grant’s Auto Body Shop, search for: grant*) This will ensure that the system returns all the possible sites with that partial name. If you search for the full name and it’s not exactly what’s already in the system you may not find your site.
- Update the information about your facility (Skip this section if all the information in Unidocs is correct and go to the next section, Electronically Submit) Owner Operator information. Update as needed.

Upload new or revised site plans, facility maps, emergency contingency plans, etc. You can upload directly from your computer pdf-formatted documents. Please use the pdf format. If you do not have the ability to convert your documents to pdf, please contact us. Delete all outdated documents.
- Electronically Submit When you have completed updating and uploading new information, go back to your site’s main manu (‘Site Main Menu’), and follow the instructions to electronically submit your updated information.
Instructions for Submitting new information via Excel or Tab-delimited format
If you cannot complete your submission via the online Unidocs website you can submit Hazardous Materials Business Plan information using an Excel or tab-delimited format. A template and field definitions are provided for preparing your Owner/Operator information and separately, for your hazardous materials inventory. You must use the format exactly as provided. Do not delete or add columns to this format without approval from the fire department.
Note: When you open the templates, save them to your computer's desktop before attempting to use.
